After completing this form, please make the change on the duty schedule, and place a copy in your supervisors mailbox. This must be completed prior to the scheduled shift. The original RA on the duty schedule will be responsible for the shift if it is not covered. If trading places on the duty schedule, a duty change form should be filled out for each duty. Following approval of your supervisor Public Safety and the professional staff on duty must be notified of all duty changes.
