TITLE PAGE:

  • Page header —first two or three words of the title that clearly
    explain the paper; allow five spaces and then place the page
    number. Page header should begin on the title page and continue
    on all subsequent pages in the upper right-hand corner.

  • Running head—Type Running head: and follow it with an abbreviated
    title; located flush left at the top of title page in all uppercase
    letters; do not exceed 50 characters. Do not include
    your name in the page header or Running head.

  • Title that accurately describes the focus of the paper;
    centered and no more than 12 words; uppercase and
    lowercase letters; if more than one line, double-space
    between the lines.

  • Author’s name is next - centered and includes middle initial.
    Omit all titles and degrees.

  • Institution’s name is placed under the author’s name and is
    also centered.
